Pelvic Radiation: Effects on the Gut, Bladder, and Skin

Pelvic radiation is a powerful and often essential part of treatment for cancers of the uterus, cervix, rectum, bladder, and prostate. Because the pelvic region is home to several closely connected systems, radiation to this area can affect more than just the cancer itself. The intestinal tract, bladder, and the delicate skin of the perineal, perianal, and vulvar region all lie close to the treatment field—and each can respond in its own way.

Just as importantly, these side effects often influence one another. Diarrhea can further irritate already sensitive skin. Urinary leakage can increase moisture and friction. Skin irritation can make sitting, walking, or using the bathroom more uncomfortable. Understanding why these side effects happen—and how they're connected—can make them feel less alarming and help you respond to them more effectively.

Effects on the intestinal tract

Radiation to the pelvic area can irritate the lining of the intestinal tract, leading to diarrhea, cramping, urgency, and increased bowel frequency. Frequent bowel movements, diarrhea, repeated wiping, and prolonged contact with stool can all contribute to redness, soreness, and irritation of the surrounding skin over time.

Stay hydrated

Diarrhea causes fluid loss quickly. Water and electrolyte-containing beverages help replace fluids and support recovery.

Choose easy-to-digest foods

Foods such as rice, bananas, applesauce, and toast are often easier on an inflamed intestinal tract. A temporary low-fiber or low-residue diet can significantly reduce symptoms for many patients when recommended by your care team.

Watch your triggers

Caffeine, alcohol, spicy foods, and artificial sweeteners can worsen intestinal irritation and diarrhea.

Ask about medications

Your radiation oncology team can recommend medications to help control diarrhea and reduce bowel discomfort during treatment.

Effects on the bladder

The bladder also sits within the pelvic radiation field and may become irritated during treatment. Radiation cystitis simply means inflammation of the bladder caused by radiation therapy.

Increased urinary frequency and urgency
A burning or uncomfortable sensation during urination
Pelvic pressure or discomfort
Blood in the urine (hematuria), which should always be reported promptly to your care team

After treatment, some people experience longer-term bladder changes, including reduced bladder capacity, persistent urgency, or urinary incontinence.

Stay well hydrated

Drinking plenty of water helps dilute urine and reduce bladder irritation.

Avoid bladder irritants

Caffeine, alcohol, carbonated beverages, and citrus products may worsen urgency and frequency.

Pelvic floor exercises

Pelvic floor exercises, including Kegel exercises when appropriate, may help support bladder control. Ask your care team whether they are appropriate for your situation.

Bladder training

Gradually increasing the time between bathroom visits may help retrain an overactive bladder.

Use incontinence products when needed

Absorbent products designed for urinary leakage help keep the skin dry.

Effects on the perineal, perianal, and vulvar skin

When the external perineal, perianal, or vulvar skin is included in the radiation treatment field, radiation can directly affect this delicate tissue, causing redness, swelling, dryness, tenderness, peeling, and increased sensitivity. Not all pelvic radiation treatments include these external areas, so not every patient will experience these skin changes.

Even when the external intimate skin is not included in the radiation treatment field, bowel changes, urinary leakage, moisture, warmth, friction, and repeated cleansing can still irritate this sensitive area. When direct radiation effects and these additional factors occur together, discomfort can become more significant.

The skin of the perineal, perianal, and vulvar region is among the most delicate on the body, and gentle skincare together with protecting the skin barrier can make a meaningful difference in comfort throughout treatment.

Cleanse gently

Use mild, fragrance-free soap and lukewarm water. Pat dry gently rather than rubbing.

Wear loose, soft clothing

Breathable cotton underwear and loose-fitting clothing reduce friction and improve air circulation.

Try a warm sitz bath

A shallow warm sitz bath may help soothe pain and calm redness, warmth, and tenderness.

Protect the skin barrier

Apply a gentle barrier product designed specifically for the external intimate skin to help protect against moisture, friction, and further irritation.

When to contact your care team

Heavy or persistent blood in the urine
Severe diarrhea causing dizziness or signs of dehydration
An inability to urinate
Open skin that is worsening rather than healing
Fever or signs of infection
